SERVICES:

Housing assistance for adults with severe and persistent mental illness and/or A.D. diagnosis.CHI has several housing options for those qualifying. These are short-term rental subsidies while the person is on the Metro Section 8 waiting list. Transitional Housing for those who are homeless and without income, addressing the housing and related needs of those returning home from prison, homeless outreach, information and referrals and subsidies for Adult Care Facility placements. Rent share is based on person’s gross income.

 
 
ELIGIBILITY:

Referrals are only accepted from mental health Case Managers or A.D. Staff. Anyone is welcome to call for information and help to access services.

Verification for an adult (single or head of household) of a severe and persistent mental illness and qualifying for mental health case management services. There are income guidelines also.

FEE POLICY:

Based on income:
Apartment Subsidies - 40% of gross income;
Transitional Housing - if no income, no charge;
if income, $250 per month or 40% of income, whichever is lower.
