SERVICES IN SIDNEY:

Case Management for PASSPORT/Care Coordination/Assisted Living Waiver for low-income seniors aged60 and over;  includes free assessment and referrals for long-term care services. Services are provided in Miami, Shelby, Darke, Preble, Logan, and Champaign Counties.

Out-patient general mental health counseling for individuals of all ages in our Sidney office. Telehealth options are available. Therapists are certified by the Ohio Department of Mental Health.

School-based counseling in Auglaize and Shelby Counties.

Parenting workshops conducted by certified Family Life Navigator.

Senior Outreach – visits and telephone support by trained volunteers

Food Source – a monthly delivery of 40-pound food boxes to supplement food needs for isolated seniors. Offered in Shelby, Miami, and Darke Counties.

Mobility Management

-RideConnect, a volunteer driver program to help bridge gaps in local transit option

-Driving with Dignity, is a program to help seniors face limitations and make appropriate adjustments.

ELIGIBILITY: For Counseling, no restrictions.  For PASSPORT/CommCare/Assisted Living, must be income-eligible and 60 years of age or older to be enrolled.  Assessment and Consultation, are free.

FEE POLICY: For Counseling:  most insurance accepted including Medicaid, Medicare, and Worker’s Compensation.  Sliding fee scale available.

PASSPORT/Assisted Living is a Medicaid waiver program.
