Non-Emergency medical evaluation by volunteer medical personnel, physicians, nurse practitioners, physician assistants, and nurses.

Follow-up care and referral to other sources of care, as needed.

Patient education and liaison with other agencies and sources of care.

Social Services: Counseling about possibilities for further care.

ELIGIBILITY: Living in Miami County

DOCUMENTS NEEDED: Photo I.D. and proof of income.

RESTRICTIONS: Only non-emergency medical needs. Children 18 and under must be accompanied by a parent or guardian.
