ELIGIBILITY: Clients must be 13 years or older. Ability to have children/no permanent sterilization.

DOCUMENTS NEEDED: Medical/Insurance card, Income Verification, Social Security card, Driver’s License, or State ID.

RESTRICTIONS: By appointment only.

FEE POLICY: Charges based on sliding fee scale based on gross income and household size.

Medicaid, Medicaid Managed Care Plans, and some private insurance are accepted.
