ELIGIBILITY:         Must be a resident of Miami County

Child:

Children from birth through age 2 who are at risk for developmental delays should call the Help Me Grow Program at (937) 440-8099.

For children ages 3 through 5 we provide through the Miami County Educational Service Center.

We provide services for children ages 6 through 21 who are determined eligible through the administration of the Children’s Ohio Eligibility Determination Instrument (COEDI).

Adult:

Be at least 16 years of age

To be found eligible through the administration of the Ohio Eligibility Determination Instrument (OEDI).

 

DOCUMENTS NEEDED:   The Eligibility Determination process will determine what documents are needed.
