Veterans in need of non-emergency transportation to Dayton VAMC

Veterans’ Services:

Prepare and file compensation or pension claim.

Assistance in completing various forms.

Assistance with VA Burial benefits.

Emergency

Financial Assistance:

A program administered by the Miami County Veterans’ Services Commission to provide, assistance when a crisis

arises. Established to assist in a basic living essential need.

ELIGIBILITY: Transportation – Veterans’ who are unable to acquire transportation to and from Dayton VA Medical

Center. Veteran with appointment(s) has first priority.

Veterans’ Services/Emergency Financial Assistance – Veterans, Veterans

Widow/Widower, Deceased Veteran’s dependent children.

DOCUMENTS NEEDED: Transportation – Copy of DD214/Discharge paper.

Veterans’ Services – copy DD214/Discharge Paper showing discharge status and documents relating to benefits

desired.

Emergency Financial Assistance – Copy of DD214/Discharge Paper; total household monthly income and expenses;

two months’ worth of pay stubs, and proof of residency.

RESTRICTIONS: N/A

HOW TO USE: Due to the volume of veterans being served, it is best to call for an appointment if you desire Veterans

Services and/or Emergency Financial Assistance.  However, we don’t discourage just stopping in, we will assist if the

schedule permits, if not we will schedule you an appointment.

Transportation:

We require you to call 48-hours prior to your appointment. Please note that all veterans going to Dayton VAMC are

transported as a group. Pickup time from your place of residence will be done to accommodate the veteran with the

earliest appointment and return will be after all appointments are completed.

Veterans’ Services:

Calling to schedule an appointment would provide us the opportunity to inform you what papers to bring to make

your application process efficient and concise. Otherwise your claim process might require two or more appointments.

pre-application forms you can complete.
