ELIGIBILITY: Referrals can be made by anyone (family, friend, physician, hospital, ECF, etc.)

Must have physician licensed in the State of Ohio.

Eligibility determined upon initial assessment and in compliance with Medicare guidelines.

DOCUMENTS NEEDED: Medicare, Medicaid card, insurance information.

RESTRICTIONS: Individualized, dependent upon client needs.
