Purpose: Hospice is a medically directed, interdisciplinary program of palliative services, a non-profit Hospice.

Our Hospice serves people with life-limiting illness and their families.

Services: Hospice emphasizes pain and symptom control wherever the patient resides by a team of professionals, including nurses, social workers, chaplains, nurse’s aides, and volunteers, with consultation with physicians.
Expert emotional and spiritual support and end-of-life care are offered. 24 Hr. On-Call availability.
Inpatient Unit availability since November 2014.

ELIGIBILITY: Any resident with a life-limiting illness.

DOCUMENTS NEEDED: None

RESTRICTIONS: If on skilled care

HOW TO USE: Call the office.

FEE POLICY: Insurance companies, including Medicare/Medicaid (as appropriate), will be billed directly.
Eligible beneficiaries will be admitted without regard to the ability to pay.
