Service & Support Administration:

*Coordinate services for individuals served

*Establish eligibility

*Assess needs for support and services

*Facilitate the development and monitor the implementation of plans

*Crisis / Emergency intervention

*Waiting List Management

*Residential Services Administration

*Remote Support Monitoring

*Developmental Center Liaison

*Training for Direct Support Staff

Children’s Programs:

*Early Intervention for ages birth – 2

*School age supports

*Recreation and Special Olympics

*Community Connection Services

*Varied programs and services for families

Adult Services:

*Community Connection Services

*Recreation and Special Olympics

*Funding for Adult Day Programming, In-Home Support, and Job Training Services through service providers.

ELIGIBILITY: Must be a resident of Miami County

Child:

*Children from birth through age 2 who are at risk for developmental delays should call 1 (800) 755-4769.

*For children ages 3 through 5 we support through the Miami CountyEducational Service Center.

*We provide services for children ages 6 through 21 who are determined eligible through the administration of the Children’s Ohio Eligibility Determination Instrument (COEDI).

Adult:

*Be at least 16 years of age

*To be found eligible through the administration of Ohio

Eligibility Determination Instrument (OEDI).

DOCUMENTS NEEDED:  Evaluation Team Report (ETR), Individualized Education Program (IEP) and medical records.
