SpringMeade Health Center in Tipp City, Ohio, provides short-term rehabilitation and recovery care, long-term nursing care, memory care, hospice care, and respite care services. Our compassionate, professional care team dedicates itself to helping patients and residents achieve their personal best, enhancing their quality of life and well-being.
The skilled nursing and rehabilitation center is located on a secure, peaceful, beautifully landscaped 40-acre SpringMeade campus. Affiliated with Premier Health and managed by AdCare Health Systems, Inc., SpringMeade is conveniently located near Interstate 75, minutes from historic downtown Tipp City, and less than 15 minutes from the medical services of Premier Health’s Upper Valley Medical Center and only 20 minutes from Premier Health’s Miami Valley Hospital.
Short-Term Rehabilitation and Recovery Care: After recovering from surgery or other hospital care, patients may not be ready to return home. The skilled nursing and rehabilitation professionals of SpringMeade Health Center’s dedicated Transitional Care Unit are ready to develop a personal plan of care to help patients recover and regain their strength, so they can return to living independently at home. Using a patient-centric approach, our highly trained team of physicians, nurse practitioners, nurses, and therapists coordinate the highest quality of care to our patients. While recovering at SpringMeade, our patients work with a consistent rehab team, providing excellent staff-to-patient ratios and advanced therapy interventions and treatments. The SpringMeade Transitional Care Unit team manages medically complex recoveries through a variety of advanced therapy and rehab services. These include:
- Physical, occupational and speech therapy – extensive therapy up to seven days a week
- Cardiac recovery-congestive heart failure program
- Orthopedic fracture and joint replacement recovery
- Pulmonary rehabilitation-respiratory care program, partnering with Respiratory Therapy at Upper Valley Medical Center
- Stroke and neurological recovery
- Wound care and recovery, partnering with Wound Care Center and Hyperbaric Services at Upper Valley Medical Center
SpringMeade Health Center Transitional Care Unit services also include:
- 24-hour RN coverage
- Intravenous (IV) therapy, central line, and total parenteral nutrition (TPN)
- Nasogastric (NG) tube, chest tube, and tracheotomy care
New Private Rehab Suites Added
In August 2018, SpringMeade Health Center completed the construction of a 12,000-square-foot addition to our Transitional Care Unit. This adds 15 private rehab suites to our existing 99-bed facility. The new suites, including a bariatric specialty suite, feature walk-in showers, and mini-refrigerators.
The new addition also includes:
- A therapy gym
- A Transitional Care Unit dining room
- A private Transitional Care Unit entrance
The dedicated SpringMeade Health Care Center Transitional Care Unit features:
- Private and semi-private rehab suites
- 24/7 wireless internet and computer access
- Complimentary cable services with flat-screen TVs
- Complimentary local phone service
- Selective and à la carte menus
Long-Term Nursing Care: The skilled nursing team at SpringMeade Health Center encourages residents to achieve their personal best, every day, through the practice of restorative care. Restorative care focuses on helping residents achieve and maintain optimal physical, mental and psychosocial function. This enhances residents’ ability to live as independently and safely as possible – and minimizes the decline that comes with aging. Through restorative care, residents are encouraged to participate as much as possible in their care and daily activities such as bathing, dressing, and transferring to and from bed or wheelchair.
SpringMeade also offers residents an activities program to help them stay engaged in hobbies and other activities they enjoy. These include music, walking and exercise programs, guest entertainers, pet therapy, outings to ballgames and theater, cookouts, and religious services. To help residents feel at home, we encourage them to bring items from home to create a home-like, familiar environment in their private or semi-private suites, which offer window views.

Memory Care: The specially trained staff of the Freedom Court Memory Care Unit at SpringMeade Health Center provide a safe, secure, supportive environment for residents with Alzheimer’s disease and other forms of dementia.
Living up to its name, Freedom Court encourages and supports residents’ independence, improving their quality of life. Freedom Court offers private and semi-private suites, a large family room, a dining room, and an outdoor area that includes a garden furnished with comfortable seating.
Our staff members build trusting relationships with residents and their families. They incorporate daily activities and socialization into residents’ care to help them achieve their personal best.
We design activities to match residents’ interests and abilities and involve residents in daily housekeeping tasks such as folding, dusting, and sweeping. These activities are therapeutic and help residents feel useful and productive.
Hospice Care: SpringMeade Health Center partners with several hospice agencies in the community to provide end-of-life care, including pain management. Hospice services provide individualized care, promoting patient/resident comfort and helping family and friends through end-of-life stages and grief.
Respite Care: Seniors can stay at SpringMeade Health Center while their caregivers are away on vacation or business – or need a brief respite from caregiving responsibilities
ELIGIBILITY: Seniors requiring 24-hour skilled or intermediate nursing care on a short-term or long-term basis, appropriate to long-term care placement via Passport and facility assessment. Short-term respite care is also available.
